Case management is a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options and services required to meet your family’s individual needs and desires
Assessment is the tool we use to determine what services will be provided to families
Families are visited on a regular basis, as often as weekly if needed, to determine the wellfare of the family: are the children doing well; are seniors who live along doing fine; do seniors have enough to eat, etc.?
School visits are made to determine if children are attending school
Meals are provided to seniors through our wellness centers, nutrition sites and home delivered meals
Violence prevention intervention is provided to youth with activities that will provide them with something positive
Programs encouraging youth to not use alcohol and drugs are a priority
Grandparents caring for their grandchildren are provided with extra support
Parenting education and supports are provided to families, youth and seniors to help them manage their lives more effectively
